Written instructions signed by parent or legal guardian will be required and will include:

  1. Child's name;
  2. Name of medication;
  3. Name of physician;
  4. Time to be administered;
  5. Dosage;
  6. Possible side effects, if known; and
  7. Termination date for administering the medication.
